STATED THAT THERE IS A MATTRESS, TRASH, AND RUBBISH IN THE BACK OF THE BUILDING. THE PERSON IN |
CHARGE WAS NOT AWARE OF THE COMPLAINT BUT STATED THAT SOMEONE IN THE AREA HAD DUMPED THE |
MATTRESS AND RUBBISH ON THEIR PROPERTY. THE PERSON IN CHARGE STATED THAT THEY WOULD NOTIFY THE |
OWNER TO HAVE THE MATTRESS AND RUBBISH PICKED UP BY THEIR WASTE MANAGEMENT COMPANY. THE |
VIOLATION IS NOTED BELOW ALONG WITH OTHER VIOLATIONS NOTED DURING THE REGULAR INSPECTION. |

OBSERVED A BULK FOOD CONTAINER OF UNPACKAGED CAKE MIX OR FLOUR THAT WAS NOT LABELED WITH THE |
COMMON NAME OF THE FOOD. PROPERLY LABEL ALL CONTAINERS OF UNPACKAGED FOODS THAT ARE NOT |
UNMISTAKABLY IDENTIFIABLE WITH THE COMMON NAME OF THE FOOD. |

readily and unmistakably recognized such as dry pasta need not be identified. |

INSIDE AND OUTSIDE OF REACH IN COOLER, AND ON CAN OPENER BLADE. CLEAN ALL EQUIPMENT AS OFTEN AS |

(A) Equipment, food-contact surfaces, nonfood-contact surfaces, and utensils. |
|
(1) Equipment food-contact surfaces and utensils shall be clean to sight and touch. |
|
(2) The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and |
other soil accumulations. |
|
(3) Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and |
other debris. |

(E) Equipment, utensils, linens, single-service articles, and single-use articles - storage. |
|
(1) Cleaned equipment and utensils, laundered linens, single-service articles, and single-use articles shall be |
stored: |
|
(a) In a clean, dry location; |
|
(b) Where they are not exposed to splash, dust, or other contamination; and |
|
(c) At least six inches (fifteen centimeters) above the floor unless the items are kept in closed packages |
on dollies, pallets, racks, and skids that are designed as specified under paragraph (II) of rule 3717-1-04.1 |
of the Administrative Code. |

(2) Clean equipment and utensils shall be stored as specified under paragraph (E)(1) of this rule and shall be |
stored: |
|
(a) In a self-draining position that allows air drying; and |
|
(b) Covered or inverted. |
|
(3) Single-service and single-use articles shall be stored as specified under paragraph (E)(1) of this rule and |
shall be kept in the original protective package or stored by using other means that afford protection from |
contamination until used. |

(A) Indoor areas - surface characteristics |
|
(1) Except as specified in paragraph (A)(2) of this rule, materials for indoor floor, wall, and ceiling surfaces |
under conditions of normal use shall be: |
|
(a) Smooth, durable, and easily cleanable for areas where food service operation or retail food establishment |
activities are conducted; |
|
(b) Closely woven and easily cleanable carpet for carpeted areas; and |
|
(c) Nonabsorbent for areas subject to moisture such as food preparation areas, walk-in refrigerators, |
warewashing areas, toilet rooms, mobile food service operation or mobile retail food establishment servicing |
areas, and areas subject to flushing or spray cleaning methods. |
|
(2) In a temporary food service operation or temporary retail food establishment: |
|
(a) A floor if graded to drain, may be concrete, machine-laid asphalt; or dirt or gravel if it is covered |
with mats, removable platforms, duckboards, or other approved materials that are effectively treated to control |
dust and mud; and |
|
(b) Walls and ceilings may be constructed of a material that protects the interior from the weather and |
windblown dust and debris. |

STATED THAT THE EXTERMINATOR VISITED THE FACILITY ON FRIDAY, DECEMBER 3, 2010 BUT HE COULD NOT |
LOCATE THE RECEIPT. |

IN ADDITION TO HAVING A PEST CONTROL COMPANY SPRAY AND SET TRAPS, GENERAL CLEANING, SANITATION, |
AND ORGANIZATION SHOULD BE IMPROVED TO ELIMINATE RODENT HARBORAGE CONDITIONS. ALSO, THE |
FACILITY SHOULD REGULARLY INSPECT THE FACILITY FOR HOLES IN THE STRUCTURE / WALLS AND FILL IN |
TO LIMIT THE ABILITY OF RODENTS / INSECTS TO TRAVEL THROUGH THE BUILDING. THE PRESENCE OF |
RODENTS AND INSECTS MUST BE MINIMIZED USING AN EFFECTIVE PEST MANAGEMENT PROGRAM. |

The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: |

|
(3) Using methods, if pests are found, such as trapping devices or other means of pest control as specified under |
paragraphs (C), (K) and (L) of rule 3717-1-07.1 of the Administrative Code; and |
|
(4) Eliminating harborage conditions. |
